What Causes Venous Leakage?

One common cause of ED, particularly in younger men, &/or men suffering from high blood pressure, high cholesterol, diabetes, ongoing stress, obesity, or peripheral vascular disease, is venous leakage, a condition where blood vessels in the veins of the penis stop working correctly. When you have an erection, the arteries in the penis are meant to expand, filling the penis with blood, then constricting and trapping the blood inside the penis (a process known as veno-occlusion) to sustain a powerful erection until you orgasm. After orgasm, arteries again widen to allow blood to flow away from the penis, back into the body, leaving you satiated, and sexually satisfied. But when venous leakage occurs, this process cannot occur, making the penis flaccid.

Let’s take a closer look at the science behind the dysfunction.

The Science Behind Venous Leakage

Your penis contains 3 long cylindrical tubes of tissue: the corpus spongiosum and a pair of corpus cavernosum (CC). The CC is housed within a tough outer covering known as the tunica albuginea (albuginea layers constitute a dense layer of tissue that encases your testes and connects to layers of fibers that surround a duct called the epididymis which transports sperm out of the testes and into the penis). The CC constitutes an interconnected network of sinusoidal spaces, surrounded by the CCSM (corpus cavernosum smooth muscle).

If you suffer from veno-occlusive dysfunction (venous leakage), damage to the CC can lead to their inability to expand and relax as they should, a result of insufficient or interrupted sinusoidal relaxation and expansion. This, in turn, disrupts the closure of emissary veins (emissary veins being veins that travel between the inner and outer layers of the tunica, which are compressed during an erection). As a result, albuginea layers become inadequately compressed and venous leak subsequently occurs, with detumescence (the medical term for ‘loss of erection’) sadly occurring. The great news is that this condition is eminently treatable.

Treating Venous Leakage

We can use penile doppler imaging (a Penile Duplex Doppler Ultrasound, or PDDU) to study blood flow in & out of your penis, studying the function of your CC arteries when erect & flaccid. Identifying blockages in the arteries or veins that supply the penis, alongside an evaluation of the health of the penis, prostate, testicles, and general pelvic area allows us to gain an intimate understanding of exactly what is causing your ED.

Following the analysis of PDDU data, we can then treat venous leakage using PRP (Platelet Rich Plasma) therapies, low-intensity extracorporeal shock wave therapy, or a combination of other therapies or treatments to restore your sexual function to optimized levels. PRP therapy usually involves the use of the P-Shot (also known as the Priapus Shot), where PRP is extracted from your own body then re-injected into your penis to promote tissue growth.
